Contacting the Surgery
Our phone lines are open from Monday to Friday between 8am and 6pm and you can contact the practice on 01273 508811.
Outside of these times you can call 111.
 
Accessibility & Parking
Patient access to Warmdene Surgery is on ground level with easy access for disabled patients.
There are parking spaces in the main car park, as well as street parking on County Oak Avenue and Carden Hill.
 
General Practice Information

Named GP
You will have been assigned a named GP whose name will have been in the registration confirmation text message.
The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
Patients do not need to see their named GP when they book an appointment with the practice.
Patients are entitled to choose to see any GP or nurse in the practice.

Viewing your Online Patient Record
Once you have registered with the practice you can set up online access to see your patient record, book appointments, view results and order repeat prescriptions.
You can see your online record either within the NHS App or Patient Access.
You can set up your own NHS App access by downloading the app onto your mobile device, or you can access the NHS App via a web browser.
You can then create an NHS Login and be verified using photo ID.
Alternatively, our Digital Ambassador can meet with you and support you with getting set up.
You can also view your online record using Patient Access, to do this you need to complete a form and provide photo ID to the practice. Please visit reception and bring your photo ID with you to get yourself set up.
 
Prospective Record Access
You will see the information in your patient record from October 2023 onwards which was when GP practices were required to automatically allow online access to patients.
This means that you will see information entered into your record after this date only.
For new patients to the practice, you will see information entered into your record from the day you are registered onwards.

Opting out of Prospective Record Access
To opt out of prospective (future) online access to your NHS GP health record, contact your GP surgery directly to request that this access be disabled. While GP practices must provide this access, you can ask for it to be turned off if it is not right for you. You have the flexibility to opt back in at any time.
 
Accessing someone else’s information
As a family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us. We do not offer this service for under 16's.
To request proxy access, please contact the Digital Ambassador
 
Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.
The NHS website has information about using linked profiles to access services for someone else.
